Credential Renewal Fees

Renewals are now due by December 31, 2024.

Please renew your credentials ONLY when all coursework and credits have been completed per the Remaining Credentialed requirements.

The fastest and easiest way to renew is to complete the form on this page and pay using PayPal (scroll down).

If you choose NOT to use PayPal, you may select ONE of the following two options:

• Download the Word version to your computer and complete (either typed or by hand). Once completed, mail it with your payment (our PO Box is on the form). Download the Word Version HERE >
